Provitamin D3 undergoes two more conversions to become the active form of vitamin D. It is first converted to calcidiol by the liver, and then to calcitriol by the kidneys and certain tissues.</p>
<p>Vitamin D deficiency is widespread, even in the US, with doctors reporting that up to 85% of their patients lack sufficient levels of the nutrient. Such rampant, ongoing vitamin D deficiency brought about a movement in the food industry to fortify foods with vitamin D. As a result, the severity of deficiencies has lessened, but the number of those affected has not abated. Inadequate levels of vitamin D have been linked to an almost endless list of diseases, from diabetes to many forms of cancer to multiple sclerosis to heart disease. Vitamin D’s essential role in calcium absorption also means that deficiencies are a leading contributor to osteoporosis. </p>
<p><strong>What are common signs and symptoms of a vitamin D deficiency?</strong></p>
<p>The most common signs and symptoms of a vitamin D deficiency relate to bone health. One manifestation is a softening of the bones, known as rickets in children and osteomalacia in adults. Muscle and bone pain and a susceptibility to fractured bones can also be telling signs of vitamin D deficiency.</p>
<p>Inadequate quantities of vitamin D can also lead to increased and prolonged inflammation. While inflammation can occur anywhere in the body, this symptom often presents as an inflammation of the gums, and over time can cause periodontal disease.</p>
<p>Other signs and symptoms of a vitamin D deficiency include a lack of energy or fatigue, sleep disturbances, and decreased immune system functioning. Vitamin D deficiencies have also been connected to symptoms of a more psychological nature, particularly mood swings, minor depression, and a tendency toward Seasonal Affective Disorder.</p>
<p><strong>What can cause a vitamin D deficiency? </strong></p>
<p>As the body requires sunlight to activate the process that synthesizes vitamin D3, deficiencies are most frequently the result of inadequate unprotected exposure to sunlight’s ultraviolet rays. It is impossible to quote a one-size-fits-all number as to how much time is needed in the sun. The answer depends on many factors: skin pigmentation, latitudinal location, season, time of day, atmospheric conditions (ozone, smog, etc.), and others.  </p>
<p>Insufficient dietary intake is the other main cause of vitamin D deficiency. Whatever is not produced via exposure to sunlight must be consumed. Fatty fish, such as salmon, mackerel, sardines, and cod are vitamin D-rich foods. In addition, shrimp, eggs, and milk are excellent sources. </p>
<p>When purchasing vitamin D-fortified foods or vitamin D supplements, it’s important to keep in mind that most use vitamin D2. Evidence suggests that this form of the nutrient cannot function as a replacement for vitamin D3. While most experts do agree that there are benefits to D2, it should not be relied on as a substitute for exposure to sunlight and animal sources of D3. </p>
<p>Diseases or disorders of the liver, kidney, and parathyroid gland can all cause a vitamin D deficiency as well. Such problems can diminish the body’s ability to convert provitamin D to its final active form, calcitriol.</p>
<p>As vitamin D is fat-soluble, consuming very little dietary fat can also cause vitamin D deficiencies. Similarly, any condition that hinders the body’s ability to absorb dietary fat, including various diseases, intestinal disorders, or chronic diarrhea, can be responsible. Obesity has also been linked to deficiencies of vitamin D, as well as the other fat-soluble vitamins. The excess of fat can make stored vitamins less accessible to the body’s metabolic processes.</p>
<p>It has also been established that aging can contribute to vitamin D deficiencies. Aging naturally results in reduced ability to use sunlight to produce cholecalciferol. Also, as the body ages, the liver and kidneys become less efficient in their roles of converting provitamin D to vitamin D. </p>
<p>Certain medications hinder the body’s ability to produce, absorb, or use vitamin D, and can therefore contribute to vitamin D deficiencies as well. Clear ties exist with some anticonvulsant drugs, Bile Acid Sequestrants (for lowering cholesterol), corticosteroids (anti-inflammatory), certain medications for treating ulcers and acid reflux, and the anticoagulant Heparin.</p>
<p>While these are the most common signs, symptoms, and causes of vitamin D deficiencies, this article is not exhaustive, nor is anything herein necessarily indicative of a vitamin D deficiency. This article is solely for informative purposes. Only a doctor can diagnose, determine a cause of, and treat any nutritional deficiency. At least 10 or 15 minutes in direct sunlight (without sunblock) every day or two and a diet regularly incorporating some of the foods listed above are the two best ways to prevent a vitamin D deficiency.</p>
